Insurance and Billing

Mt. Diablo Memory Center is a Medicare provider.  We will bill Medicare and your secondary insurance carrier for any services provided at our clinic. Medicare Advantage plans do require pre-authorization before rendering services in most cases. We will work with your medical provider to facilitate authorization.

We are contracted providers with John Muir Medical Group, Sutter East Bay Medical Foundation, and Hill Physicians Medical Group.  For these insurance plans, pre-authorization must be requested by your referring physician prior to scheduling services at our clinic.

We are not participating providers in any private insurance plans (e.g. PPO).  Services will be provided utilizing your out-of-network coverage.  We can assist with determining the extent to which our services will be covered by your insurance. We also welcome cash-pay clients and accept payment by personal check or credit card; payment plans are available.
